Notice to Auto Insurance Patients

Effective January 1, 2004, Front Range Center for Brain & Spine Surgery changed its policy regarding auto insurance.

We are happy to bill any auto insurer your request, but it will be up to you, the patient, to follow up on your account with the auto insurer. If you have any questions, you may contact our billing department at (970) 493-1292.

If you cannot provide auto insurance information, you will be asked to pay at the time of service regardless of your health insurance coverage. If you do not have health insurance, we will expect you to pay 50% of the billed charges at the time of service—no exceptions.

  • Please bring to your appointment:
  • Insurance card (for both auto and health insurance)
  • Claim number
  • Date of injury
  • Adjustor’s name and phone number
  • Insurance companies’ names, addresses, and phone numbers

We do not accept third-party auto insurance.

Notice to Out-of-State Auto Insurance Patients
