Important Information for New Patients

Concerning Your X-rays, Other Radiologic Exams and Insurance Referrals

In order for your appointment to be as beneficial and efficient as possible, it is essential that you make arrangements to have any pertinent x-ray, CT scans, MRI, or other radiologic exam of your brain and/or spine present at the time of your appointment with us.

You do not need to bring your films with you if they were done at Poudre Valley Hospital, Medical Center of the Rockies, The Imaging Center, North Colorado Medical Center, Summitview Medical Commons Radiology, McKee Medical Center, Open MRI of the Rockies, Skyline Imaging, Sterling Regional Medical Center or Mountain Vista Imaging.

Patients who have an open and active Colorado, Wyoming, or Nebraska Workers’ Compensation claim must have a written referral from a representative of the orkers’ Compensation insurance carrier.

Also, it is your responsibility to have the necessary referral in place for your appointment with us. Please check with your insurance company to determine if your insurance coverage requires a formal, written referral to see a specialist. We require written referrals from a licensed healthcare professional for patients whose primary health coverage is Medicaid.

Please call one or two days before your scheduled appointment to make sure films and referrals are here. Your appointment may need to be re-scheduled if your films are not here at our office.

You may call us at (970) 493-1292 or (800) 458-0306.
